Attention Parents/Guardians:  

By registering your child, you agree that they have permission to attend Straight Up Summer Camp on July 9 - 13.  You have read and agree with all the information contained on this webpage.  Your child also has permission to participate in any recreation at his/her own discretion.  You realize that there is a risk associated with any activity and will assume full responsibility for the welfare of your child.  You will not hold FBC Liberty City, nor any of its sponsors, leaders, staff, agents, or representatives responsible in case of injury or illness.  You also recognize that FBC Liberty City has a current medical release form on file for your child, so that in the event of an emergency, church sponsors may seek medical and/or surgical treatment.
