Building Usage
Anyone interested in using any of our buildings at FBC Liberty City, must fill out a Building Usage Form and submit it to the church office. All events/activities must be approved by our church administrator before they can go onto the church calendar.

Medical Consent Form
For the safety of our children, it is a requirement at FBC Liberty City, that all children and youth 18 years or younger must fill out a Medical Consent Form before they participate in any FBCLC activities.
This form must be notarized. FBCLC will notarize this form, free-of-charge, Monday - Thursday. Please call the church office to make sure a notary is available (903) 984-4494.
